Medication Administration in School
The following individuals are authorized by law to administer medications to students in schools:
In order for your child to receive medication in school from the School Nurse the following must be in the School Health Office:
1. Written permission for administration of medication in school signed by the parent or legal guardian.
2. Written orders from an individual licensed to prescribe medication (with signature) including:
a. Pupil’s name
b. Name of medication
c. Diagnosis, purpose of medication
d. Proper timing and dosage
e. Possible side effects
f. Date when medication will be discontinued
g. Activity restrictions if necessary
h. A statement that the pupil is physically fit to attend school and is free of contagious disease
i. Is the child authorized to self-medicate
